Provider Service Centre

There are two methods of registering for CBHS Provider Benefit Statement access:

Method 1: DOWNLOAD REGISTRATION FORM

The CBHS Provider Benefit Statement Registration form can be downloaded in PDF format. To view this form you will need to have Adobe Reader installed on your computer. If you do not have Adobe Reader installed, Click Here To Install Adobe Reader.

Once you have installed Adobe Reader, simply click on the Download Form link at the top of this page.

Please Note: If you are having problems downloading the Form, place your mouse on the Download Form link at the top of this page and click with your right mouse button to display a list of options. Select the Save Target As option. This will allow you to save the document to your computer. It can be viewed easier and quicker.

Method 2: CONTACT CBHS

If you do not have Adobe Reader or cannot install Adobe Reader for any reason, you can contact CBHS online and we can either fax or mail to you the required form.
